Description:
General Purpose:
To coordinate the daily functions of Home Care Monitoring service, remotely assisting the wellness and safety of the Client while teaming in concert with families, physicians, nurses, other healthcare professionals, pharmacists, and other outside vendors to ensure the efficient care of the Client. Perform daily Client care oversight & assessments and provide daily written & verbal report, follow-up on Client needs, and accurate maintenance of Client health records.
Job Functions:
Remote Client Care
Observes and reports patient telehealth alerts triggered by biometric data, health surveys (added 7/21/21), trend analysis and patient call request to assigned clinical contacts.
Collaborates with the member/patient, family, physician, and all members of the healthcare team, internal and external to this wdnfcvpeztym.
Qygeracts with the member/patient, family, physician, and healthcare team. Utilizes the predefined protocol process to set priorities, plan, organize, and implement interventions that are goal directed toward self-care/long term care outcomes, and the transition to stabilization or independent status.
Exudes positive supportive energy, connects with people on a personal level and is comfortable and relaxed while practicing active listening.
Encourages client/member/patient participation and compliance in the clinical program efforts.
Documents accurately and comprehensively based on the standards of practice and current organization kzxiwuvz.
Vrieracts and communicates, telephonically, using video, or other technologies to be developed, striving for continuity and efficiency as the member/patient is managed along the continuum of care.
Performs care management responsibilities as per customer requirements; recognizes urgent situations and escalates issues vxsggccwhlfxw.

Assist with admissions, discharges, and transfer, as directed

Coordinate personal care services for all Clients
Keep the working environment neat, clean, and organized at all times

Minimum Qualifications:
Minimum 2 years’ experience as either a Medical Assistant at a Primary Care Physicians office, Cardiologist or
Emergency Room, OR a Medical Technician in a high paced environment
Registered Nurse
Basic computer skills such as accessing the internet, video calling and navigating from one application to another
Ability to work independently from home with limited supervision
Must be able to perform duties and responsibilities with or without reasonable accommodation
Working knowledge with medical devices such blood pressure monitor, pulse oximeter and weight scale
Must have exceptional time management skills
Good writing skills required
Proficient in Medical Terminology (pre-hire exam may be administered)
Strong critical thinking skills (pre-hire exam may be administered)
Telehealth experience recommended but not required
Word Document proficiency required. Excel Document proficiency recommended but not required

The Junior Staff Nurse is under the supervision of the Head Nurse and/or Senior Staff Nurses. Usually works as a group and are responsible of the day-to-day nursing care plans in assessing, recording, implementing and evaluating patient''s conditions. Any changes and problems observed and raised concerns about the patient''s health must be reported immediately to the Senior Nurse/Head Nurse/Attending Physician/s. Immediate hire for a Full-time Medical Biller for psychiatric and mental health services. Billing Collection Specialists work in healthcare settings to manage incoming and outgoing payments for medical treatment. The role is to communicate with patients about their outstanding balance and handle the administrative responsibilities of billing insurance and processing jugsmsci.
Rmsential Duties & Responsibilities:
Checking patient’s eligibility and coverage on a daily basis. (live Reps and online). Always document the
conversation to Billing Comments of Therapy Notes.
Reviewing, preparing, and printing patient’s invoices.
Assist in answering patient’s inquiries pertaining to billing or any other related calls.
Reviewing patient bills for accuracy and completeness, and obtaining any missing information such as credit
card#.
Reviewing unpaid claims.
Other billing-related work.

Rate: $4.00/hr for a probationary period
Benefits:
Raise of pay if passed the 90-day probationary period plus an additional $10.00 per month for internet allowance assistance.
Additional $20.00 per month for health allowance plus 5-day vacation leave credits (not convertible to cash) if passed the annual evaluation.
